Job summary

Agape Care Group is seeking a Registered Nurse to evaluate patients, develop care plans, and coordinate communication with patients’ families and the care team. You will lead the care team to ensure compassionate, high-quality hospice care and support for patients facing life-limiting illnesses.

The role emphasizes patient-family communication, collaboration with interdisciplinary teams, and ongoing professional development within a supportive leadership structure.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care.
• Education: Graduate of an accredited school of nursing with a current state license as a registered nurse.
• Experience: 2+ years of nursing experience in a clinical care setting (hospice experience preferred).
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively.

Responsibilities

• Evaluate patients and create care plans.
