Job summary

Signature HealthCARE is seeking an experienced RN for the Elizabethton Rehab & Wellness Center to provide direct nursing care, administer medications, and supervise nursing assistants during shifts.

You will contribute to individualized care plans, ensure safety, accuracy in documentation, and collaborate with an interdisciplinary team to promote resident well-being and quality outcomes. We offer strong benefits and growth opportunities.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents’ physical and emotional status; notify physician and responsible party of changes.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
