Job description

**Summary:** Reporting to the Patient Care Manager, this position actively participates as a member of the Patient Centered Medical Home Team by collaborating with physicians and multidisciplinary team members to ensure the delivery of high-quality patient care. Performs appropriate treatments as ordered by physician in an accurate and timely manner. Promotes the mission and philosophy of providing quality health care and related services to the medically underserved. Ensures compliance with all corporate and clinical policies and procedures, as well as the mission and values of the organization.**Primary Responsibilities:** • Ensures all actions, job performance, personal conduct and communications represent the organization in a highly professional manner at all times. • Promote and participates in Patient Centered Medical Home activities in order to achieve and maintain level 3 NCQA PCMH recognition. • Direct, supervise and evaluate the nursing care provided to patients by LPNs and/or CMA/RMA. • Actively participate in nursing care provided to the patients by leading morning huddles, performing chart prep and utilizing programs such as Azara and dot phrases to ensure patient care consistency. • Delegate job responsibilities to LPNs and/or CMA/RMA as appropriate. • Triage patient calls for acute medical conditions and assess what level of care patients may require utilizing Clear Triage. • Conduct Medicare wellness and nurse visits and provide additional patient care as needed. Conduct follow-up phone call within 48-72 hours to assess the patient’s status, schedule follow-up appointment, confirm patient has picked up their medications, and address any new barriers. • Review patient medications, update medication lists, and add prescription information to the medication list. Administers medication including oral, Sub Q, and IM. Chart prescription refills in the patient record or phones prescription(s) into a pharmacy under the direction of the patient’s provider and per protocol. • Contact patients to report normal and abnormal test results within an appropriate time frame. Prioritize results to ensure patients with emergent results are contacted expeditiously. • Assists with vital signs, interviewing patient and completing health history, documentation in patient record, prepare patient for examination and/or procedures, updating medication profiles. • Participates in mandatory annual in-services, certification and educational programs to maintain competency and maintain certification. • Ensures HIPAA compliance at all times by adhering to patient rights, safety, privacy, and confidentiality procedures and protocols. • Performs other duties as assigned.**Essential Job Functions:** The demands described below are representative of those that must be met by an employee to successfully perform the essential functions of the job. Must be able to understand, adhere to and communicate routine, factual information and apply oral and written instructions or other information, in a timely manner. • Capable of physically transporting items weighing 5 to 25 pounds. • Use upper extremities to press against something with steady force in order to thrust forward, downward, or upward for up to ten hours per day. • Substantial movements of the wrists, hands, shoulders and/or fingers for sustained periods of time. • Must have the visual acuity to accurately detect and read computer screens, distinguish color coding, and discern fine print and/or normal type size print. • Capable of meeting attendance requirements. • Ability to move about the facility to greet and room patients, access supplies, and other duties that require movement on foot for prolonged periods of time. • Constantly operates a computer, office equipment and/or other medical equipment to collect and monitor patient data, report patient data and patient needs.
