Job summary

Adaptive Home Health, LLC seeks an experienced Registered Nurse to perform comprehensive home health assessments at patients' homes. You will develop care plans, administer infusion/ wound care, and coordinate with physicians and families.

The role requires 3+ years of nursing experience, a Texas RN license, and CPR/BLS certification. Will travel across multiple Texas locations, with a full-time schedule and potential productivity-based pay.

Qualifications

• Current Texas RN license and 3 years clinical nursing experience.
• Experience with wound care and infusion therapy.
• CPR certification required.
• Familiarity with Homecare Homebase EMR preferred.

Responsibilities

• Perform comprehensive home health assessments and document via Homecare Homebase.
• Develop and update care plans; communicate with physicians and care team.
• Administer medications and treatments, including infusion therapy and wound care.
• Provide direct patient care and educate patients and families.
• Supervise nursing staff (LVNs & HHAs) and participate in case conferences.

Pay:

$64.10 per point/30 points per week

$100k to $133k per year

Seeking elite Registered Nurses with experience in wound care and infusions! Previous home health experience highly preferred.

This position is full time with the expectation of 30 productivity points completed per week. The clinician will be paid per point and has the opportunity to earn additional pay if the clinician elects to perform more than 30 productivity points per week.

Position requirements:

Registered Nurse is responsible for comprehensive assessments in the patient's home, including the completion of skilled documentation through Homecare Homebase

• Evaluate patient and home health environment and performs skilled home health assessments; clinical documentation and visit notes; care plan development; communication with physician, care team members, patient and family.
• Comfortable treating wounds and performing infusions
• Serve as the patient's advocate and maintain ongoing communication with physician, referral sources, caregivers and facility management to facilitate coordination of care.
• Administer medications and treatments as prescribed by physician. Provides direct patient care including infusion therapy, wound care, teaching, disease management.
• Provide direct supervision of nursing clinical care team members (LVN's & HHA's).
• Participate in regular case conference and in-services

Requirements:

• Current Texas Registered Nurse (RN) license
• Three years of clinical nursing experience
• Current Texas driver's license
• CPR Required
• Experience with healthcare software (Homecare Homebase preferred)
• Competency to carry out any skilled procedure that the physician orders that is within the scope of the Nurse Practice Act
