Job summary

One Stop Recruiting is seeking a compassionate Registered Nurse (RN) in North Chicago, IL to provide in-home and community care. The role welcomes new grads and experienced RNs, with a Monday–Friday, 8:00 AM–5:00 PM schedule.

Salary ranges from $70,000 to $80,000 per year, with additional compensation for IPU shifts and on-call. Benefits include health insurance, PTO, retirement plan, malpractice coverage, a company phone, and more.

Qualifications

• Active, current RN license in Illinois.

Responsibilities

• Conduct comprehensive nursing assessments in home and community settings.
• Develop, implement, and evaluate individualized care plans.
• Provide pain and symptom management and timely interventions.
• Educate patients and families on disease progression and care options.
• Coordinate care with physicians, NPs, social workers, and other team members.
• Perform home safety assessments and caregiver interventions.
• Document assessments and patient responses in the EHR within 24 hours.
• Participate in IDT meetings, QAPI, and on-call rotations.

Registered Nurse (RN) – North Chicago, IL

Overview

Our home health and hospice team in North Chicago is seeking a compassionate Registered Nurse (RN) to provide care to patients in their homes and communities. New grads and experienced RNs alike are welcome to apply.

Position Offers

• Schedule: Monday–Friday, 8:00 AM–5:00 PM

Key Responsibilities

• Conduct comprehensive nursing assessments of patients’ physical, emotional, and psychosocial status in home and community settings, monitoring for changes in condition
• Develop, implement, and evaluate individualized care plans focused on symptom control, comfort, and quality of life
• Provide pain and symptom management, including medication administration, monitoring, and timely intervention
• Educate patients and families on disease progression, care options, medications, and what to expect at each stage of care
• Coordinate care with physicians, NPs, social workers, chaplains, aides, and other team members
• Perform home safety assessments and implement interventions to promote comfort and caregiver safety
• Document assessments, interventions, and patient responses accurately and promptly within 24 hours of each visit in the electronic health record (EHR)
• Participate in interdisciplinary team (IDT) meetings, case conferences, quality assurance and performance improvement (QAPI), in-services, and on-call rotations

Qualifications

• Active, current Registered Nurse (RN) licensure in Illinois
• Strong communication, organization, and compassion, with the ability to work independently while collaborating with an interdisciplinary team
• Travel throughout surrounding areas required; valid driver’s license and reliable transportation required
