Job summary

Signature HealthCARE in Carrollton, KY is seeking an experienced RN to join our team and provide direct resident care, including medications and wound care, while supporting a strong care team. This role emphasizes safety, dignity, and quality of life within a 78-bed rehab and wellness setting.

You will lead shifts, supervise CNAs, and contribute to individualized care plans in partnership with the interdisciplinary team. Strong documentation and regulatory knowledge are essential.

Qualifications

• Active RN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum 1 year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Excellent verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) ensuring comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift per policies.
• Monitor and observe residents’ status; notify physician of any change in condition.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Complete accurate clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges.
• Participate in quality assurance activities, staff education, and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.

How you Will make a difference

What you Need to make a Difference

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
