Job summary

Sympathy Home Health Care Inc in Bedford Heights, Ohio is seeking a Registered Nurse to provide in-home skilled nursing care. The role involves assessing patients, developing care plans, and coordinating with Clinical Supervisors to ensure timely, compassionate care in patients' homes.

Required RN license with at least 1 year of home care experience; BLS is preferred. Work location is in person, with opportunities for full-time, part-time, or PRN schedules.

Qualifications

• Must hold a valid RN license in the state of employment.
• At least 1 year of home care experience.
• Competent in performing home care comprehensive assessment.
• BLS certification is preferred.

Responsibilities

• Assess and evaluate patient needs and set mutually agreed goals with patients.
• Develop and update patient care plans; revise as changes occur and at recertification.
• Complete admission paperwork and care plan submission to Clinical Supervisor.
• Provide Skilled Nursing services including assessment, procedures, teaching and training per Plan of Care.
• Schedule visits and notify if the schedule needs alteration.
• Report significant findings to patient's physician and Clinical Supervisor.
• Submit SN visit notes and home health aide supervisory notes per policy.
• Perform all OASIS time point assessments for Medicare recertification.
• Coordinate with colleagues to implement patient care plans.
• Administer medications and monitor for side effects or reactions.
• Document changes in patient conditions and communicate with supervisor.

Job Summary

Sympathy Home Health Care Inc is located in Bedford Heights, Ohio. We provide home health care services including skilled nursing and home health aides. We are looking for a Registered Nurse to care for our patients and facilitate their speedy recovery on a regular basis in their residence.

QUALIFICATIONS

A state professional Nurse License.

At least a year experience in home care. competent in performing home care comprehensive assessment.

RESPONSIBILITIES

Assess and evaluates patient needs/problems, identifies mutually agreed upon goals with patients.

Develops a patient care plan that addresses identified patient needs and objectives. Updates care plans on an ongoing basis; revises and resolves patient issues and goals as changes occur and/or at recertification.

Completed admission paperwork and patient care plan submitted to Clinical Supervisor per agency policy following the admission including completed and signed admission paperwork.

Provides Skilled Nursing services including Assessment, evaluation, procedures, teaching and training activities as outlined in the patient Plan of Care.

Provides Skilled Nursing visits according to visit schedule and notifies agency of need to alter schedule in any way.

Reports significant findings to patient's physician and Clinical Supervisor as they occur.

Submits completed skilled nursing visit notes; communication notes and home health aide supervisory notes per agency policy on designated days as requested by Clinical Supervisor.

Submits change orders per agency policy.

Performs all OASIS time point assessment per Medicare Criteria and submits recertification paperwork per agency policy and procedure.
