Job summary

Siena Skilled Nursing and Rehabilitation Center, part of Generations Healthcare, seeks an RN to join the Behavioral Health Unit. The RN will provide direct care, supervise staff, and ensure compliance with federal, state, and local standards.

The role emphasizes teamwork, proper documentation, and maintaining high-quality resident care in a behavioral health setting. Full-time with a 4/2 rotation on the NOC shift is offered, along with a comprehensive benefits package.

Qualifications

• Must possess a Nursing Degree from an accredited college or university or be a graduate of an approved RN program.
• Must possess a current, unencumbered RN license to practice in this state.
• Preferred minimum 1 year of experience in a psych or locked facility or hospital setting.
• Knowledge of nursing practices and regulations pertaining to nursing care facilities.

Responsibilities

• Cooperate with resident services to coordinate nursing care on the STP.
• Participate in facility surveys and inspections.
• Review and update the resident discharge plan.
• Plan the nursing services portion of discharge plan as needed.
• Admit, transfer, and discharge residents as required.
• Complete injury/incident reports and forms.
• Maintain charting and documentation as required.
• Protect patient privacy and report violations.
• Complete and file required record-keeping forms at admission/transfer/discharge.

Position Overview

The primary role of the RN in our Behavioral Health Unit is to provide direct nursing care to our residents, and to supervise and assist the day-to-day nursing activities performed by Mental Health Specialists. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the STP Unit Director, the Director of Nursing Services and/or Nurse Supervisor to ensure that the highest degree of quality care is maintained at all times.

Essential Duties

• Cooperate with other resident services when coordinating nursing services to ensure that the resident’s total regimen of care is maintained on the Special Treatment Unit (STP).
• Participate in facility surveys (inspections) made by authorized government agencies as may be requested.
• Periodically review the resident’s written discharge plan.
• Participate in the updating of the resident’s written discharge plan as required.
• Assist in planning the nursing services portion of the resident’s discharge plan, as necessary.
• Admit, transfer, and discharge residents as required.
• Complete injury/incident reports, as necessary.
• Completing medical forms, reports, evaluations, studies, charting, as necessary.
• Agree not to disclose resident’s protected health information and promptly report suspected or known violations.
• Complete and file required record keeping forms/charts upon the resident’s admission, transfer, and/or discharge.

Qualifications

• Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved RN program.
• Must possess a current, unencumbered, active license to practice as an RN in this state.
• Preferred, at a minimum, 1 year of experience in a psych and/or locked facility or hospital setting.
• Must demonstrate knowledge and skills necessary to provide care appropriate to the behavioral and nursing needs of the residents served.
• Must be a supportive team member, contribute to and be an example of teamwork and team concept.
• Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
• Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
• Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, and procedures that are necessary for providing quality care.
• Must be enthusiastic about caring for behavioral residents and have the desire to be part of a dynamic team.
• This position requires the successful completion of a background check, pre-employment physical, and drug screening as part of the hiring process.
