Job summary

Alternate Solutions Health Network in Akron, OH seeks a Registered Nurse (RN) to provide one-on-one skilled care within patients' homes and supported by an interdisciplinary team.

You will monitor conditions, administer medications, educate families, and document care in accordance with state guidelines. This full-time role offers guaranteed base salary, uncapped bonus opportunities, mileage reimbursement, and a comprehensive benefits package.

Qualifications

• Registered Nurse with current license in the state of employment.
• Minimum one-year experience as an RN in an acute care setting; home care experience preferred.
• Valid driver's license and auto insurance in your name as a driver.

Responsibilities

• Observe and monitor patient conditions and perform assessments to develop an individualized care plan.
• Administer medication as prescribed and help decrease re-hospitalizations by prioritizing high-risk visits.
• Manage multi-disciplinary care while promoting continuity of care with admissions, transfers, and discharges.
• Educate patient and family on disease processes using teach-back methods.
• Complete clinical documentation following agency protocol and Medicare guidelines.
