Job summary

NHC HomeCare Aiken is seeking an RN to provide in-home care and coordinate patient plans in a fast-paced environment. The role requires an active RN license and at least one year of nursing experience, with a preference for home care experience.

The nurse will assess, implement, and evaluate care plans, collaborate with the interprofessional team, and educate patients and caregivers on health management and disease prevention.

Qualifications

Unencumbered RN license in the state where the agency is located or a compact state.
Minimum of 1 year experience as an RN/professional nurse.
Home care experience is desirable.

Responsibilities

Utilizes the nursing process to identify and achieve patient goals: assessment, planning, implementation, and evaluation.
Assesses factors affecting health to develop a comprehensive care plan and attain desired health outcomes.
Collaborates with the interdisciplinary team to provide personal care, medical care and rehabilitation.
Teaches patient/caregiver in health care and disease management and documents ongoing needs.
