Registered Nurse - Rehabilitation (RN) Summary

• Provide direct patient care in a rehabilitation setting, supporting recovery and functional improvement.
• Develop, implement, and evaluate individualized care plans based on patient assessments and goals.
• Collaborate with a multidisciplinary healthcare team, including physicians, therapists, and case managers.
• Maintain detailed and accurate patient records in compliance with facility policies and regulatory requirements.

Duties & Responsibilities

• Administer medications and treatments as prescribed, monitoring for adverse reactions or side effects.
• Perform comprehensive patient assessments to identify needs and monitor progress.
• Facilitate patient mobility and rehabilitation exercises, following prescribed therapy programs.
• Advocate for patient needs and communicate effectively with family members and care team.
• Participate in discharge planning and ensure continuity of care after patient discharge.
• Adhere to all safety protocols, infection control measures, and ethical standards in nursing practice.
• Support and mentor less experienced staff as needed to promote a collaborative environment.

Qualifications & Requirements
