Job summary

The Rehabilitation Institute of South San Antonio, operated with Lifepoint Health, seeks an experienced Registered Nurse (RN) for rehab care on a full-time night shift. You will assess patients, implement nursing interventions, and document care plans while coordinating with interdisciplinary teams.

Requirements include a current state RN license and a bachelor’s degree in nursing. BLS required within 30 days; ACLS and PALS within six months.

Qualifications

• Current state RN license and bachelor's degree from an accredited nursing school.
• Basic Life Support certification within 30 days of hire.
• ACLS and PALS within six months of hire.
• One year of Rehab and/or Med Surg experience preferred.
• ASLS certification preferred.

Responsibilities

• Performs accurate patient assessments and identifies needs.
• Initiates appropriate nursing interventions and documents care.
• Delivers care aligned with patient condition and age and coordinates with care team.
• Maintains timely documentation of medical necessity and updates care plans.

How You'll Contribute

A Registered Nurse (RN) who excels in this role:

• Accurately performs patient assessments and identifies patient needs
• Identifies and initiates appropriate nursing interventions
• Provides care appropriate to condition and age of the patient
• Performs timely and appropriate documentation relating to medical necessity in the medical record
• Responsible for completion and revision of the Interdisciplinary Care Plan for each patient
• Performs timely and accurate QI assessments
• Strong interpersonal and communication skills required.
• Ability to work collaboratively with interdisciplinary teams.
• Attention to detail in clinical documentation is essential.
• Essential Functions Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, as well as other medical care treatment. Documents patient care given. Monitors, records, and communicates patient condition as appropriate to care team, physician, patient, and family. Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation. Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs. Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances. Works as an advocate for the physical and emotional well-being of the patient.
