HealthLinc in Valparaiso, IN is seeking a grant-funded Registered Nurse to join the Program Care Team as a Nurse. You will continuously evaluate patient care systems, collaborate with the healthcare team to ensure continuity, and report patient progress.

This role works closely with medical staff to support patient outcomes. The position requires Indiana RN licensure, CPR certification, and preferably 1 year of ambulatory or public health nursing experience.

Qualifications

  • Current CPR certification is required.

Responsibilities

  • Review dashboard to monitor patient vitals.
  • Perform assessment and data collection with the healthcare team.
  • Identify unsafe environments and coordinate emergency planning with providers.
  • Assist with on-site patient calls and device utilization concerns.
  • Schedule on-site appointments with the care team nurse or pharmacist.
  • Assist patients and families to function at their optimum level.
  • Collaborate with Program Director on device monitoring and tracking.
  • Participate in continuous quality improvement activities.
  • Ensure continuity of patient care across healthcare settings.
  • Promote effective communication among patients, families, and team members.
  • Document patient contact in the EHR and keep records up to date.
  • Initiate and document patient and family teaching and responses.
  • Coordinate services with other health-related agencies and resources.
  • Obtain referrals to physicians and specialists.

As the Program Care Team Nurse, you will continuously evaluate patient care systems, collaborate with the healthcare team to ensure continuity of patient care, and report patient progress. This position will work closely with the medical staff.

JOB RESPONSIBILITIES:
  • Review dashboard to monitor patient vitals.
  • Performs assessment, data collection, obtain, review and analyze information in collaboration with the healthcare team members and others as appropriate.
  • Identifies unsafe patient environment and intervenes responsibly. Discusses situations with Medical Providers, Pediatricians and Nurse Practitioners for development of emergency plan for patient and family regarding their health care.
  • Assist with management of patient calls with concerns of their vitals or utilization of device.
  • Assist with scheduling on site appointments with patient care team nurse or Pharmacist.
  • Assists patients and families in functioning at their optimum level.
  • Collaborate with the Program Director on any monitoring and tracking of devices
  • Participates in continuous quality improvement activities by evaluating patient care systems that may include standards, protocols, and documentation.
  • Collaborates with the healthcare team to ensure continuity of patient care throughout the healthcare settings.
  • Promotes effective communication among healthcare team members including patient, family and significant others.
  • Utilize EHR to document patient contact.
  • Keeps care coordination records up to date.
  • Initiates, implements, and documents patient and family teaching and documents their response to teaching.
  • Assists in implementing and carrying out continuous quality improvement programs and activities.
  • Coordinates services for patients and families utilizing other health related agencies and resources.
  • Obtains referrals to other physicians and specialists.
  • Participates in program meetings.
  • Follows approved protocols/standing orders for patient care.
  • Ensures that a productive and efficient patient flow occurs.

QualificationsREQUIRED QUALIFICATIONS:
Education/Training
  • Bachelor of Science in Nursing (not required but highly preferred)
  • Registered Nurse licensed in the State of Indiana
  • Current CPR Certification
Experience
  • At least 1-year ambulatory or public health nursing experience (not required but highly preferred)
