Veterans Health Administration seeks a registered nurse to provide gender-specific Veteran care and comprehensive care coordination in Primary Care. You will manage patient panels, deliver wellness coaching, and develop individualized care plans based on nursing assessments.

The role requires valid nursing licensure in the United States, English proficiency, and eligibility for licensure; opportunities exist for temporary appointment during bridge programs as needed.

Formación

• Graduation from an accredited nursing program with licensure in the U.S.
• English proficiency required for direct patient care per VA standards.
• State license or eligibility for licensure; temporary appointment possible during bridge program.

Responsabilidades

• Provides gender-specific Veteran care and care coordination within the Primary Care Department.
• Manages assigned panels and uses registries to ensure evaluation and access to patient panels.
• Delivers health promotion, education, and coaching on wellness and chronic care management.
• Forms care plans based on nursing assessments and delegates care appropriately.
• Maintains flexibility to improve care with supervision and guidance as needed.
