• Provide competent, safe, and compassionate care based on established standards.
• Formulate the initial plan of care with the provider and collaborate on a comprehensive patient-centered plan.
• Collect, assess, and document patient healthcare data.
• Involve patient/family and interdisciplinary team to identify a realistic and measurable plan of care.
• Prioritize care based on collected data.
• Document assessment and reassessment in medical records.
• Implement interventions in the coordination of patient care in a safe and timely manner.
• Educate the patient/family regarding health status and established plan of care.
• Evaluate the patient's progress towards expected outcomes and involve the interdisciplinary team in the evaluation process.
