Qualifications

Responsibilities

• Performs nursing care per physician orders and plans care for designated patients.
• Establishes and maintains trust with patient and family during care delivery.
• Assess patient outcomes and adjust care plans accordingly.
• Engages patients and families in care decisions and education.
• Develops individualized, outcomes-based care plans with education components.
• Uses evidence-based interventions for diagnoses and conditions.
