Qualifications

• BSN or Associate Degree matriculated into a bachelors program.
• Current CT RN license.
• Basic Life Support (BLS).
• ACLS and PALS.

Responsibilities

• Provide patient-centered care as the coordinator of care across the continuum.
• Independently uses and documents the nursing process to plan, deliver, and evaluate goal-focused care.
• Ensure coordination of thorough and complete, population-specific continuum of care planning.
• Collaborate with the health care team to support complex care planning.
• Lead unit-based projects and participate in shared governance and quality initiatives.
• Mentor and educate peers, promoting evidence-based practice and professional development.
