Job summary

CN Guidance & Counseling Services is hosting a Virtual Job Fair for Registered Nursing roles focused on acute care within the Intensive Crisis Stabilization Center. The RN will work with a multidisciplinary team to provide direct patient care, implement treatment plans, and coordinate discharge planning in accordance with clinical guidelines.

The role requires NY-licensed RN with 1 year community mental health experience or 2 years in a mental health setting.

Qualifications

• Must be licensed as a Registered Nurse by a state board
• 1 year of community mental health experience or 2+ years in a mental health setting
• Experience with children/adolescents is preferred

Responsibilities

• Coordinate client services including scheduling appointments and procedures
• Conduct medication reconciliation and monitor pharmacological effects
• Perform vitals and health assessments
• Ensure completion of medical assessments and coordinate with psychiatrists
• Educate patients about medications, risks and side effects
• Enter and maintain accurate documentation in the EHR
• Conduct shift change reports to ensure continuity of care
• Share data with relevant individuals to support delegated decisions

JOB DEFINITION:

The Registered Nurse is responsible for providing short-term acute care to individuals presenting to the Intensive Crisis Stabilization Center with a mental health/substance use crisis. The Registered Nurse provides direct patient care and implements treatment plans under the direction of the Nurse Manager and medical staff, following established clinical guidelines, policies, and procedures. The RN will work with a multidisciplinary team to ensure safety, promote recovery, and assist with treatment planning and discharge coordination.

EDUCATIONAL REQUIREMENTS:

Must be currently licensed as a Registered Nurse by the NY State Department of Education, and possess an Associate's (if awarded prior to December 18, 2017), Bachelor's or Master's Degree in Nursing from an accredited program*.

EXPERIENCE REQUIRED:

One (1) year of community mental health experience and/or at least two (2) years of mental health experience specifically working in a mental health setting.

Previous experience working with children and adolescents in a professional setting is highly preferred.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• 1.Responsible for the coordination of client services to include scheduling of outside clinic appointments, lab appointments, obtaining random urine analysis (UA), intramuscular (IM) and subcutaneous injection administration, when needed
• 2.Conducts medication reconciliation, when appropriate, or during transitions of care.
• 3.Conducts vitals and health assessments.
• 4.Ensures the completion of all medical assessments for each client, including coordinating additional services with the psychiatrist.
• 5.Monitors the pharmacological effects of prescribed psychotropic medications and notifies the psychiatrist of their reported effectiveness.
• 6.Working in conjunction with the psychiatrist and nurse practitioner, is responsible for the coordination of patient education efforts to include the benefits, risks and possible side effects of prescribed medication. This education effort is not limited to home visits but may also include the coordination of various groups to include but not limited to, medication, wellness, etc.
• 7.Responsible for entering the documentation in EHR.
• 8.Responsible for conducting shift change reports and ensure continuity of care through accurate communication and documentation.
• 9.Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress
• 10.Complete QI Training during onboarding and participate in QI refresher training as needed.
