Job summary

The Registered Nurse PACT Outpatient position is part of Nursing Ambulatory Care at the West Haven VA Healthcare System, located on the West Haven, CT campus. This is a full-time, 40-hour per week role.

The RN provides direct Veteran care, care coordination, and health promotion within the Primary Care Department, supervising LPNs and nursing assistants as appropriate and contributing to evidence-based outpatient nursing practice.

Qualifications

Associate Degree Nursing (ADN), Bachelor of Science in Nursing (BSN), or Master of Science in Nursing (MSN) must be fully accredited by ACEN or CCNE.

Responsibilities

Provide competent, evidence-based gender-specific primary care to Veterans.
Oversee licensed vocational nurses/nursing assistants as applicable.
Deliver direct Veteran care and care coordination within Primary Care.
Manage patient panels, use registries, and ensure access to care.
Promote health education, wellness coaching, and chronic care management.
Contribute to nursing education, orientation, and quality improvement.
Collaborate with interdisciplinary team to sustain evidence-based practice.

This Registered Nurse PACT (Patient Aligned Care Team) Outpatient position is in Nursing Ambulatory Care at the West Haven VA Healthcare Systems, located at the West Haven, CT. campus. This position is full-time at 40 hours per week.

Key Responsibilities

The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) is responsible for providing competent, evidence-based gender-specific care to primary care patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department.

Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels. This nurse provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management. The nurse will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization.

Demonstrates leadership in delivering and improving holistic, primary care through collaborative strategies with others. Evaluates practice in an ongoing process, based on best evidence. Provides peers with informal constructive feedback for improvement. Fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Contributes professional outpatient nursing perspective in discussions with the interdisciplinary team.

Partners with others to effect change and produce optimal outcomes. Supports colleagues through knowledge sharing to provide safe, quality outpatient nursing care. Shares educational findings, experiences, and ideas with peers. Questions PACT clinical practices for the purpose of providing evidence-based care. Participates in activities and strategies to sustain an evidence-based-practice culture.
