Qualifications

What does it look like to work on a PACT Team as a Registered Nurse:

PAC Team is a wrap-around team of different professionals that all support a common goal. Our culture is one of transparency, support, and cohesiveness. PACT allows for autonomy and the freedom to treat our patients as human beings first, providing low to no barrier, community-based care that allows for improved outcomes in one of the most vulnerable populations. This position allows nurses to get to know their patients and the environment they live in meaningfully. This position is caring for those with severe, persistent mental illnesses. No psychiatric experience is required, as we are happy to train and promote your own growth.

Starting Pay Range:

RN - $43.27 to $47.02/hour

LPN - $31.72 to $34.29/hour

Summary:

This member of the Community Medicine team is responsible for conducting physical assessments, assessing physical health needs, making appropriate referrals to community physicians, providing management of, and providing a range of limited wound care/cleaning, assessment of symptoms, and support services.

Essential Job Responsibilities:

  • Provide service coordination (case management) for community members, including coordinating and monitoring the activities of the individual treatment team (ITT).
  • Assume primary responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals and plans in collaboration with the client and the ITT.
  • Providing individual supportive therapy and symptom management.
  • Educating and supporting clients’ families.
  • Advocating for clients’ rights and preferences.
  • Consult with community agencies and families to maintain coordination in the treatment process.
  • Document client progress to maintain a permanent record of client activity according to established methods and procedures, ensuring immediate changes are made in the treatment plans as clients’ needs change.
  • Participate in daily staff organizational meetings and treatment planning review meetings.
  • With other registered nurses on the team, develop, revise, and maintain medical treatment and medication policies and procedures, including transcribing, administering, evaluating, and recording medical services/referrals made.
  • Under the direction of the team manager and director, organize and manage the system of providing medical outreach and integrating medical services tightly into clients’ individual treatment plans.
  • Under the direction of the team Director and in collaboration with other team members, manage outreach and medical supplies.
  • In collaboration with the team director, coordinate, schedule, and administer medical assessments of client physical health, making appropriate referrals to community physicians for further assessment and treatment.
  • Provide direct clinical services to clients on an individual, group, and family basis in the office and in community settings to teach health management skills.
  • Provide individual and group treatment in the office and in community settings in a stage-based treatment model that is non-confrontational, considers interactions on mental illness and substance abuse, and has client-determined goals.

Non-essential Job Functions:

  • Support agency mission, goals, and objectives.
  • Attend staff and planning meetings as scheduled.
  • Maintain communication and cooperative working relationships both within the agency and community at large.
  • Other duties as assigned by Supervisor.

Education, Licensing and Experience:

  • Registered Nurse license or Licensed Practical Nurse.
  • Two to four years of experience working with chronically mentally ill adults, and current knowledge of psychopharmacology.
  • Must know basic wording of mental illness symptoms and knowledge of psychological medications.
  • Drivers License, car insurance, and reliable transportation preferred.
