Job summary

City of Hope is seeking a Registered Nurse for its Outpatient Clinic in Georgia. The RN will provide nursing and case/disease management services across the full continuum of care, coordinating with an interdisciplinary team to ensure optimal clinical and financial outcomes.

The successful candidate will assess patients, educate families, document in the EMR, and support treatment options. A BSN is preferred, with BSN completion within three years, and BLS certification within 30 days of hire.

Qualifications

• Current Georgia RN license from an accredited college.
• BSN required or enrolled within 6 months of hire and BSN completed within three years from hire date.
• BLS (AHA certification) within 30 days of hire.

Responsibilities

• Assess, coordinate, plan, monitor, implement and evaluate the patient’s plan of care for outpatient oncology patients.
• Provide professional nursing services to support the physician in direct and indirect care.
• Document assessments, interventions, and patient education in the electronic medical record.
• Educate patients and families, support informed decisions, and connect to ancillary services.
• Collaborate with the interdisciplinary team to ensure coordinated, high‑quality care.
• Adhere to facility policies, procedures, and regulatory requirements; assist with central line care and procedures as needed.
• Respond to patient calls/drop‑ins promptly to ensure efficient service delivery.

To demonstrate support of the hospital mission and vision by providing nursing, case management/disease management services to clients and families to help in the coordination and management of individuals’ health needs through the full continuum, as needed. The Centers Registered Nurse demonstrates the ability to assess, coordinate, plan, monitor, implement and evaluate the patient’s plan of care while ensuring and facilitating the achievement of optimal quality, clinical and financial outcomes. Performs assessment of clinical and life issues for the patients of CTCA. Provides professional nursing services in support of the physician, in the direct and indirect care of patients.

The successful candidate:

• Must be able to work Monday-Friday, 8a-4:30p.

• Assessment and Coordination : The nurse assesses patients' clinical and life issues, coordinates care plans, and collaborates with the interdisciplinary healthcare team to ensure optimal patient outcomes.
• Disease Management in Oncology : With expertise in oncology, chemotherapy, and its side effects, the nurse monitors and addresses symptoms, educates patients and families about their disease and treatment plans, and addresses quality of life issues.
• Documentation and Communication : Timely and thorough documentation of assessments, interventions, and patient education in the electronic medical record is crucial. The nurse communicates effectively with the healthcare team and patients to ensure coordinated care.
• Patient Education and Support : The nurse provides patient education, supports informed decision-making regarding treatment options, and ensures access to ancillary services and resources.
• Team Collaboration and Customer Service : Serving as a resource and facilitator, the nurse promotes communication and collaboration among healthcare team members, patients, and families. They strive to provide excellent customer service, increase patient satisfaction, and achieve positive outcomes.
• Adherence to Policies and Procedures : The nurse performs tasks according to facility policies, regulatory requirements, and best nursing practices, including central line care, medication administration, and procedure assistance.
• Efficient Service Delivery : The nurse ensures efficient delivery of services by covering for team members, responding to patient calls and drop-ins promptly, and meeting hospital turnaround time goals.
