Job summary

Live Well Homecare in Scranton, PA is seeking a registered nurse to deliver patient care in line with state nursing standards. You will assess, plan, implement, and evaluate care, including treatments, medications, and diagnostic studies, while addressing physical, psychological, social, cultural, and spiritual needs.

The role includes coordinating individualized care plans, educating patients and families, discharge planning, and collaborating with physicians and the health team.

Qualifications

• Graduate from a nursing specialty training program.
• Preferred experience in operating room scrub role.
• Ability to work in a clinical environment, adapt to shift rotations, and respond promptly during on‑call situations.

Responsibilities

• Formulates and coordinates individualized care plans, including patient and family education and discharge planning.
• Evaluates intervention effectiveness and adjusts care as needed.
• Collaborates with physicians, ancillary staff, and interdisciplinary team members to meet patient needs.
• Delegates tasks based on team members' skills and patient acuity, promoting teamwork and safety.
• Participates in quality improvement initiatives, evidence-based practice, and safety programs.
• Supports unit decision-making and contributes to a positive work environment.
