The San Francisco Campus for Jewish Living in San Francisco's Excelsior neighborhood is seeking a Registered Nurse to assess, plan, implement, and evaluate individualized care for residents. You will supervise staff and ensure compliance with clinical and regulatory standards.

This On-Call/Per Diem PM shift involves administering medications, communicating changes to the care team, and delivering high-quality nursing care in a collaborative, mission-driven environment.

Qualifications

• Graduate of an accredited School of Nursing.
• Current, active Registered Nurse (RN) license in the state of California.
• Current CPR certification.
• BSN preferred; two years of acute or long-term care experience preferred.
• Ability to communicate clearly, both verbally and in writing.
• Skilled in clinical assessment, supervision, and collaboration with interdisciplinary teams.

Responsabilités

• Develop and initiate individualized care plans based on resident needs upon admission.
• Continuously assess residents and update care plans accordingly.
• Communicate significant changes in residents' conditions to the health care team.
• Administer medications and treatments safely and accurately.
• Serve as a clinical resource and provide guidance to LVNs and CNAs.
• Direct and supervise daily nursing care delivery on the unit.
• Document all care concisely and timely.
• Assume charge responsibilities and supervise unit operations in the absence of the head nurse.
• Ensure adherence to regulatory guidelines, including Title 22.

POSITION SUMMARY

As a Registered Nurse, you will be responsible for the comprehensive assessment, planning, implementation, and evaluation of individualized care for each resident. You will play a critical role in ensuring quality care, supervising staff, and maintaining compliance with clinical and regulatory standards.

KEY RESPONSIBILITIES

• Develop and initiate individualized care plans based on resident needs upon admission.
• Continuously assess residents and update care plans accordingly.
• Communicate significant changes in residents' conditions to the appropriate health care team members.
• Implement appropriate nursing interventions, including discharge planning and resident education as needed.
• Administer medications and treatments safely and accurately.
• Serve as a clinical resource and provide guidance to LVNs and CNAs.
• Conduct initial shift rounds to establish a baseline assessment and provide oversight of resident care.
• Direct and supervise daily nursing care delivery on the unit.
• Document all care provided concisely, accurately, and in a timely manner.
• Assume charge responsibilities and supervise unit operations in the absence of the head nurse.
• Ensure adherence to all regulatory guidelines, including Title 22.
• Foster a professional and collaborative working environment among all health care team members.
• Accept reassignment as needed to ensure resident care needs are met.
• Perform other related duties as assigned.

JOB DETAILS

• Pay Rate: $62.37/hr
• PM shift differential: $2.85/hr
• Weekend shift differential: $2.50/hr
• Hours: On-Call/Per Diem
• Shift Schedule: PM Shift, 3 PM - 11:30 PM
• Days Off: Monday - Friday
• Working Days: Saturday and Sunday
• Unit: F2

REQUIRED QUALIFICATIONS

• Graduate of an accredited School of Nursing.

• Bachelor of Science in Nursing (BSN) preferred.
• Two years of acute or long-term care experience preferred.

PHYSICAL REQUIREMENTS

• Ability to lift up to 25 pounds
• Ability to stand, walk, bend, and assist residents for extended periods
• Ability to use proper body mechanics and assistive devices safely
