Job summary

The MetroHealth System is seeking a Nurse Home Visitor to deliver comprehensive nursing care to women and families enrolled in the NFP program. You will plan, deliver, and document care in alignment with NFP and MetroHealth policies, while observing and reporting on patient conditions.

This role requires Ohio RN licensure, BLS certification or ability to obtain within 90 days, and flexible availability including some evenings and weekends; travel to Denver, CO for education sessions may be

Qualifications

• Graduate of an accredited school of professional nursing.
• Current RN licensure in the State of Ohio.
• AHA Basic Life Support for the healthcare provider certification or obtain within 90 days of hire.
• Knowledge of nursing theory and practice across biological, physical, psychosocial, and medical sciences.
• Excellent written and verbal communication skills.
• Basic computer skills.
• Reliable transportation to travel to patients\' homes and MetroHealth locations.
• Ability to work a flexible schedule including some evenings and weekends.
• Ability to travel to Denver, CO for all face-to-face education sessions.
• Preferred: home visiting experience; two years maternal/child health, public health, home visiting or mental/behavioral nursing; bilingual (English and Spanish).
• Physical demands: movement throughout day; sitting, standing, stooping, bending, ambulating.

Responsibilities

• Provide comprehensive nursing services to women and families eligible for the Nurse Home Visitor Program.
• Plan, deliver, document and evaluate nursing care in accordance with NFP guidelines and MetroHealth policies.
• Observe, assess and report on patient conditions; maintain accurate, timely documentation of care.

Shift: 8a-4:30p

Summary

The Nurse-Family Partnership (NFP) Nurse Home Visitor is responsible for providing comprehensive nursing services to women and their families eligible for the NFP Program. Maintains the highest standards in clinical nursing practice and adherence to the NFP model, and to policies, procedures, guidelines, and standards of NFP and of The MetroHealth System. Plans for, delivers, documents and evaluates nursing care of patients in an assigned area in terms of their individual needs, according to provider's orders and in conformance with approved and recognized nursing standards of care, recognized nursing techniques, procedures and administrative policy. Observes, assesses and reports on the condition of patients. Maintains accurate and timely documentation of care reflecting patient's condition, treatment, and response to care. Upholds the mission, vision, values, and customer service standards of The MetroHealth System.

Qualifications

Required:

• Graduate of an accredited school of professional nursing (or a program seeking accreditation).
• Current registered nurse licensure in the State of Ohio.
• Current AHA Basic Life Support for the healthcare provider certification or obtains within 90 days of hire.
• A thorough knowledge of nursing theory and practice, including basic knowledge relating to nursing such as biological, physical, psychosocial, and medical sciences and their applications.
• Excellent written and verbal communication skills.
• Basic computer skills.
• Reliable transportation to travel to patients' homes and MetroHealth locations based on operational needs.
• Ability to work a flexible schedule including some evenings and weekends.
• Ability to travel to Denver, CO for all face-to-face education sessions.
• Preferred: Home visiting experience preferred. Two years recent experience in maternal/child health, public health, home visiting or mental/behavioral nursing preferred. Bilingual (English and Spanish).
• Physical Demands: May need to move around intermittently during the day, including sitting, standing, stooping, bending, and ambulating.
