Job summary

Wellpath in Vermont seeks an experienced Registered Nurse (RN) for a full-time night shift at the VT DOC Southern SSCF facility. The role involves direct patient care, nursing documentation, and collaboration with the care team.

Responsibilities include implementing medical plans, administering medications, and ensuring safety and compliance. Must have RN licensure in the state, CPR certification, and at least 1 year of clinic experience is preferred.

Qualifications

  • Graduate from an accredited school of nursing.
  • Current licensure as a Registered Nurse in the state of employment.
  • Must maintain CPR certification.

Responsibilities

  • Assist in assessing and planning individual treatment programs, consulting with Charge Nurse and staff as needed.
  • Implement medical plans by administering medications, obtaining diagnostic tests, and assisting with minor procedures as needed.
  • Count controlled substances and implement clinical and technical aspects of care according to policies.
  • Document nursing encounters using SOAP notes and attend mandatory trainings.
  • Communicate information to nursing staff, physicians, and supervisory personnel as necessary.

How you make a difference

As a Registered Nurse(RN), your primary responsibilityis toprovidepatient care using the nursing process of assessment, planning,implementation,and evaluation.You will work under the supervision of Charge Nurse(s) to provide patient education and activities in accordance with your competencies and education.This role requires excellent communication and critical thinking skills to ensureoptimalpatient outcomes.


Key Responsibilities

  • Count controlled substances and implement clinical and technical aspects of carein accordance withestablished policies, procedures, and protocols.

  • Document nursing encounters using the SOAP form of charting as required by policy and attend mandatory staff meetings and training.

  • Communicate information to nursing staff, physician, health care unit supervisory personnel or otherstaffas necessary.


Qualifications & Requirements

Education


  • Graduate from an accredited school of nursing.


Experience


  • Prefer a minimum of one (1) year clinic experience


Licenses/Certifications


  • Have and maintain current licensure as a Registered Nurse within the state of employment.

  • Must be able to obtain and maintain CPR certification.
