Job summary

Solvista Health is hiring Registered Nurses to support the Regional Assessment Center in Salida, Colorado. The RN will coordinate, provide, and oversee professional nursing care, including assessment, treatment planning, and care coordination with a strong emphasis on patient-centered outcomes.

Full-time, part-time, and PRN positions are available. Sign-on bonuses are offered: $6000 for full-time and $3000 for part-time roles.

Qualifications

  • RN degree. Completion of nursing training from an accredited school of nursing. BSN preferred.
  • Minimum of three years professional nursing experience.
  • Experience working with mental health and substance abuse populations in an outpatient clinic setting preferred.
  • Current state registered nurse license and CPR certificate.

We are Hiring Registered Nurses to work at our Regional Assessment Center in beautiful Salida, CO!

PT, PRN and FT positions available

FULL-TIME SIGN-ON BONUS: $6000 for Full time payable 1k at the end of 12 months, 2k at the end of 24 months, and final payment of 3k at the end of 36 months of employment.

PART-TIME SIGN-ON BONUS: $3000 for part time payable 1500k at the end of 6months, 1500k at the end of 12 months of employment.

General Purpose

The Registered Nurse (RN) is responsible for the coordination, provision and oversight of professional nursing care for patients including assessment, treatment, care planning, medical care evaluation, care coordination and evaluation. The RN is the primary liaison between patients, staff and other members of the patient’s care team.

Essential Duties/Responsibilities
  • Demonstrates quality, confidential and compliant clinical nursing care.
  • Determines the needs of the patient assessing the urgency of connecting the patient to the needed resources based on the patient’s condition.
  • Obtains patient consent for care and ensures patient confidentiality.
  • Obtains and documents health history, chief complaint, vital signs, and risk factors
  • Assesses disease condition.
  • Provides patient care and care coordination to meet the needs of the client.
  • Administers prescribed medications.
  • Collaborates with the patient to determine patient goals, interventions, and a process of monitoring progress towards goals.
  • Educates patient and families about health status, maintenance, and management of conditions.
  • Serves as the liaison between the medical and behavioral health staff.
  • May provide supervision to nurses and care coordination staff.
  • Documents patient assessment, goals, intervention, and goal completion in the EHR.
  • Serves as key problem-solving resource for staff on patient issues.
  • Helps develop and implement regulatory compliant policies, procedures, quality assurance plans, and work standards.
  • Maintains skills to provide direct care.
  • Participates in multidisciplinary teams to improve patient care processes and outcomes.
  • Performs other duties as assigned.

Summary of Compensation

The base compensation represents the entry point of the range. Actual salaries will vary and may be above or below the range based on factors including but not limited to location, experience, education and performance. The salary is just one component of Solvista Health's total compensation package for employees.

Education or Formal Training:

Work Schedule
  • Full-time: Three 12-hour shifts per week

Night Shift Pay Differentials

Base pay range for Registered Nurse position is $38.33-42.66

Sunday - Thursday nights: +$7.00/hour (Total pay range $45.33 - $49.66)

Friday - Saturday nights: +$10.00/hour (Total pay range $48.33 - $52.66)

Working Conditions and Other Conditions of Employment

Working Environment:

This job operates in a fast-paced, professional 24-hour-a-day environment. This position has high physical and mental requirements. It is highly interactive due to constant communication with patients, families, and interdisciplinary teams. And emotionally intense due to the exposure to trauma, severe mental illness, and sometime aggressive behavior. The position may require travel to conferences, meetings and branch locations on a regular or intermittent basis. Work may involve moderate exposure to unusual elements, such as extreme temperatures, dirt, dust, fumes, smoke, unpleasant odors, and/or loud noises. In the health center environment, there is potential for contact with blood-borne pathogens and communicable diseases, as well as potential for contact with dissatisfied or abusive individuals.

Physical Requirements
  • 12-hour shifts
  • Must be able to lift 25lbs or more
  • Standing and walking for long periods of time during shifts
  • Responding to emergencies quickly (codes, behavioral crises)
  • Escorting or physically supporting patients when needed
  • Ability to safely move patients off the floor from a fallen position
  • Must be able to egress patients through exit doors safely
  • Must be able to physically maneuver yourself through doors that do not have automatic door opener
  • Be able to assist a client in ambulating on the unit or ability to push a patient in a wheelchair on unit or to hospital
  • Fine motor skills
  • Ability to remain calm under pressure
Psychological and Mental Demands:
  • High emotional demand due to caring for patients with severe mental illness (schizophrenia bipolar, etc)
  • Exposure to suicidal ideation, self-harm, and trauma histories

Conditions of Employment:

  • Annual TB, federally required drug screening, and Influenza vaccination or compliance with policy and procedure.
  • Required possession of a valid state driver's license.
