Job summary

Beloit Health System at Beloit Regional Hospice is seeking a Registered Nurse Night Shift Case Manager to join our team in Wisconsin. The position covers 3rd shift (4:30p-8a) with a 7 on/7 off schedule and a sign-on bonus.

Responsibilities include coordinating interdisciplinary care for patients and families, documenting needs, and educating caregivers per hospice policies. RN license in WI or IL is required with ability to obtain the other state license within 30 days.

Qualifications

  • Graduate of accredited nursing program; BSN preferred.
  • Current RN license in Wisconsin or Illinois; ability to obtain license in the other state within 30 days.
  • Valid driver's license and reliable vehicle.
  • Ability to work independently with minimal instruction.
  • Maintain knowledge of state and federal hospice regulations and RN scope of practice.

Responsibilities

  • Communicate with multidisciplinary team and primary care physician about patient condition.
  • Develop and update an interdisciplinary plan of care with patient/family input.
  • Support physical, social, emotional, and spiritual needs of patient/family.
  • Document patient/family needs, goals, and interventions in the medical record.
  • Educate primary caregivers, family, volunteers, and caregivers to provide care as indicated.
  • Supervise on-site evaluation of CNAs delivering care every 14 days.
  • Assess and admit patients and complete paperwork per hospice policies.
  • Maintain positive, cooperative communication with patients, families, and colleagues.
  • Participate in agency in-services and community education to promote growth.

The Registered Nurse Night Shift Case Manager is responsible for the provision of care for all patients/family on call needs based on the nursing process. The Registered Nurse Night Shift Case Manager is responsible for the coordination of services provided by the Interdisciplinary Team assuring that the patient and family needs are met, and care is provided consistently with federal and state regulations, Beloit Health System/Beloit Regional Hospice policies and procedures, and hospice standards of practice.

Responsibilities
  • Communicate regularly with the multidisciplinary team, including the primary care physician, regarding patient's condition.
  • Develop a plan of care for each patient involving the patient/family and interdisciplinary team and update plan of care when patient condition changes.
  • Be attentive to physical, social, emotional, and spiritual aspects of patient/family experience, supporting directly or consulting with other team members to meet needs.
  • Document patient/family needs, goals, and interventions accurately in the medical record.
  • Document all patient encounters prior to end of shift for continuity of care.
  • Instruct and educate primary caregivers, family, volunteers, and employed caregivers to provide care as indicated.
  • Supervise and document on-site evaluation of certified nursing assistant delivery of care every 14 days.
  • Communicate to any unusual or potentially problematic needs/concerns.
  • Assess and admit patients, develop initial Interdisciplinary plan of care and complete paperwork in accordance with Beloit Regional Hospice policies and procedures.
  • Communicate in a positive, responsive, cooperative manner with patients, families, team members, and all others you encounter as a representative of Beloit Regional Hospice.
  • Anticipate and meet patient and family needs. Participate in agency in-services and community education opportunities as requested to promote professional growth and understanding of hospice care.
Job Requirements
  • Graduate of accredited nursing program. BSN preferred.
  • Current RN license in Wisconsin or Illinois, with the ability to obtain a license in the opposite state. Must apply within 30 days of hire.
  • Valid driver's license and reliable vehicle.
  • Ability to work independently with minimal instruction.
  • Assessment skills, flexibility, dependability, and teamwork skills are imperative.
  • Maintain current knowledge of state and federal hospice regulations, and the RN Scope of Practice for Wisconsin and Illinois.
  • Assist in providing 24-hour, 7 day-a-week coverage to patients/families.
  • Maintain ethical conduct and keep confidential personal and medical information about patients.
  • Adhere to established hospice and health system, rules, and regulations.
  • Participate in educational programs and continually seek for personal development.
  • May be called in event of need.
  • Physical effort includes walking, standing, stair climbing, bending and stooping, lifting, reaching, and sitting at long intervals.
  • Manual and finger dexterity and eye/hand coordination
  • Ability to see and hear at a sufficient level to perform job duties with or without reasonable accommodation.
  • Ability to speak clearly and distinctly and to interact positively with patients, peers, and the public.
  • Ability to carry 15-30 pounds, read, write and follow oral and written instructions.
