GT Independence is seeking a CoordinatedCaregiving Registered Nurse in North Carolina to oversee safe medication administration and clinical coordination for participants in the Coordinated Caregiving Program.

You will work with Case Management Entity RNs and community partners to ensure regulatory compliance, accurate MAR documentation, and staff training on medication safety and regulatory requirements.

Qualifications

Responsibilities

The CoordinatedCaregiving (CCG)Registered Nurse plays a critical role in ensuring the safe and effective administration, monitoring, and documentation of medications for individuals served in GT Independence's North Carolina's Coordinated Caregiving Program. In addition, the RN is responsible for the oversight, delegation, and training of specialized skilled cares, ensuring that all clinical supports are delivered in accordance with best practices, regulatory standards, and the unique needs of each individual served.

This position collaborates with Case Management Entity RNs, Case Managers (CMs), and community partners to support clinical needs and medication safety in accordance with GT Independence policies, DHS regulations, and Coordinated Caregiving guidelines.

Responsibilities

Clinical Coordination & Assessment

  • Partner with the Case Manager to obtain and verify current medication lists from Primary Care Providers.
  • Conduct in-home assessments to identify clinical needs and ensure compliance with GT Independence policies.

Medication Management

  • Maintain accurate Medication Administration Records (MARs).
  • Monitor MAR documentation for accuracy and compliance.
  • Educate CCG staff on medication administration, monitoring, storage, handling, disposal, and regulatory requirements.

Training & Oversight

  • Provide initial and annual medication administration and monitoring training.
  • Train staff on the Six Rights of Medication Administration.
  • Review MARs monthly and address documentation concerns as needed.
  • Oversee medication preparation and documentation for Leave of Absence situations.
  • Provide training and oversight for specialized skilled care tasks.

Documentation & Compliance

  • Ensure all medications, including prescriptions, OTCs, and supplements, are documented in MARs.
  • Maintain current consents, disclosures, and medication profiles.

Quality Assurance

  • Collaborate with Operations and Compliance teams to ensure adherence to CARF standards, Medicaid requirements, and GT Independence policies.
  • Participate in audits, quality reviews, corrective actions, and medication safety initiatives.

Community Collaboration & Education

  • Partner with community organizations to identify staff training needs.
  • Educate individuals served, families, and guardians on medication use, side effects, and informed decision-making.

Qualifications

  • Experience delivering compassionate, individualized care, focused on supporting people through a wide range of health journeys and personal goals.
  • Current and unencumbered RN license in the state of North Carolina
  • Strong understanding of medication administration protocols and regulatory compliance.
