Job summary

Advocate Health’s AMG Chicago OB/GYN team is seeking a Registered Nurse for a full-time, on-site role in Hazel Crest, IL. Schedule is Monday to Friday, 8:00am–4:30pm, with 40 hours per week and no weekends.

The nurse will coordinate care across the continuum, educate patients and families, and collaborate with a multidisciplinary team to improve outcomes in obstetrics/gynecology. Ideal candidates hold an Illinois RN license, a Bachelor’s Degree in Nursing, and about five years of nursing

Responsibilities

• Provides continuity of care by ensuring smooth transitions between care settings and coordinating with the multi-disciplinary team.
• Completes a nursing assessment to identify the patient's individualized needs and collaborates with multidisciplinary team and other departments to develop an interdisciplinary plan of care.
• Supports the patient and family by providing education, addressing psychosocial needs, advocating, providing continuity of care and identifying barriers to care.
• Navigates patients and family members through the diagnosis, treatment and follow-up of the patient throughout the continuum of care.
• Provides expert clinical advice to the multidisciplinary team in designing collaborative teaching plans/programs and in planning/providing patient/family education and discharge planning including wellness and health maintenance within designated specialty.
• Coordinatestimely, seamless, evidence-based care across the care continuum resulting in best patient outcomes.

Navigates patient and family members through the diagnosis, treatment, and follow-up of the patient throughout the continuum of care ensuring smooth transitions between care settings. About This Location

Our main location is in the Medical Office Center at 17850 S Kedzie Ave., Suite 3100, in Hazel Crest, IL, with additional locations in Orland Park, South Holland, and Frankfort, IL. Our practice includes seven providers, four OB/GYN physicians, one gynecologist, two Advanced Practice Nurses and a dedicated social worker. We provide comprehensive obstetric and gynecologic care to women of all ages. In office procedures include colposcopy, endometrial biopsy, LEEP and hysteroscopy.

Physical Requirements and Working Conditions

• Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday.

• Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies.

• Must be able to: push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present.

• Must have functional speech, vision, hearing, and touch ability to use fine hand manipulation skills for IV insertion and other procedures/functions.

• Maneuver foot pedals on carts or machines.

• Perform physical safety interventions such as patient restraint and verbal de-escalation, if needed.

• May be exposed to chemical and hazardous waste as well as blood and body fluids and communicable disease. Therefore, protective clothing and equipment must be worn as necessary.

• Must be able to respond quickly to changes in patient and/or unit conditions.

• Physical Abilities Testing may be required.

• Additionaldepartment specific physical requirements may be identified for unique responsibilities within the department by the nurse leader.
