PruittHealth is seeking a Registered Nurse Navigator to join our care team in Atlanta. The RN will coordinate post-acute care, guide residents and families, and collaborate with providers to ensure seamless transitions.

The ideal candidate holds an active RN license in the state, demonstrates strong communication, and is committed to compassionate, patient-centered care within a supportive nursing environment.

Qualifications

• Experience coordinating post-acute care transitions (preferred).
• Strong communication and teamwork skills.
