Job Description

Long-Term Care (LTC) Registered Nurses (RNs) at a Veterans Affairs Medical Center (VAMC) provide compassionate, comprehensive nursing care to veterans with chronic illnesses, disabilities, or those in need of end-of-life care. These RNs typically work in Community Living Centers (CLCs), which function like skilled nursing or nursing home units within the VA system.

Key Responsibilities

• Deliver direct patient care including assessments, medication administration, wound care, and monitoring of vital signs.
• Develop and implement individualized care plans in collaboration with interdisciplinary teams.
• Support patients with activities of daily living (ADLs), such as bathing, dressing, feeding, and mobility assistance.
• Monitor changes in residents’ physical and mental health and promptly report concerns to the medical team.
• Provide emotional support to veterans and their families, particularly in palliative and hospice care settings.
• Maintain accurate documentation in accordance with VA policies and electronic health record standards.
• Participate in quality improvement initiatives, infection control, and safety protocols.

Schedule

• Unlike ICU where it’s self-scheduling, the CLC will create the schedule for the LTC RNs 30 days in advance
• 12-hour shifts
• 3x Days
• 3x Nights
• Rotating Weekends and Holidays
• 36 hours per week

RN Requirements

• Must have BSN
• Must have at least one year of recent experience in LTC/SNF
• AHA BLS, VA will not accept non-AHA certifications.

Qualifications

• Currently licensed Registered Nurse (RN) in state of employment
• Previous experience as a Registered Nurse is preferred
• Excellent interpersonal and client care skills
• Valid driver’s license
• First aid/CPR certification required
