BrightStar Care of Concord, Lexington & Woburn is seeking an experienced Home Care Registered Nurse (RN) Manager to lead our clinical operations. The right candidate will join the Leadership Team and receive support to grow toward a Director of Nursing role within 1–3 years.

You will own quality and reliability of our home care services, manage Plan of Care, supervise staff, ensure regulatory compliance, and partner with physicians to deliver exceptional patient care across Concord, Lexington,

Qualifications

• Active RN License in the state of operation.
• 2-3 years of home health, hospice, or skilled nursing experience.
• IV Infusion Therapy proficiency including TPN.
• Sales and business development mindset for referrals.
• CPR Certification.
• Leadership and management experience supervising nurses and aides.
• Care plan development and oversight.
• Quality assurance and risk management experience.
• Strong problem-solving and decision-making abilities.
• Valid driver's license, car, and auto insurance.
• On-call availability and emergencies readiness.

Responsibilities

• Develop and implement patient care plans in collaboration with physicians and other healthcare providers.
• Conduct in-home assessments and ensure that patient care meets regulatory and company standards.
• Oversee compliance with state and federal home health regulations, including documentation and reporting requirements.
• Supervise and manage the home care nursing team, including hiring, training, and evaluating staff performance.
• Provide guidance and support to nurses and caregivers to ensure the highest level of patient care.
• Manage scheduling and ensure adequate staff coverage for all home care cases.

Registered Nurse (RN) Job Responsibilities:

Registered Nurse (RN) Qualification and Requirements: Must Haves:

• Active RN License – Must hold a valid and unrestricted Registered Nurse (RN) license in the state of operation.
• Clinical Experience – At least 2-3 years of experience in home health, hospice, or skilled nursing.
• IV Infusion Therapy Expertise – Proficient in administering and overseeing IV infusions, including medications, hydration, and TPN (total parenteral nutrition)
• Sales & Business Development Mindset – Ability to attract and retain clients and collaborate with referral partners
• CPR Certification
• Leadership & Management – Strong ability to supervise, train, and mentor nurses, aides, and caregivers.
• Care Plan Development – Skilled in creating and overseeing patient care plans, ensuring high-quality outcomes.
• Quality Assurance & Risk Management – Experience in auditing, incident reporting, and maintaining compliance with policies.
• Problem-Solving & Decision-Making – Ability to handle staffing issues, patient concerns, and operational challenges quickly and efficiently.
• Reliable Car, Driver's License and Auto Insurance (Required)
• Flexibility & Availability – Comfortable with on-call responsibilities and responding to emergencies

Nice to Haves:

• Home Health or Private Duty Nursing Experience – Familiarity with BrightStar Care’s business model is a plus
