The Lakes Home Care, Inc. seeks a dedicated Registered Nurse to join our Home Health team in Florida. You will apply the nursing process and deliver patient care under the direction of the Director of Clinical Services, using clinical judgment to prioritize and manage a patient load.

This role offers flexible work hours, a 1 patient:1 nurse ratio, tailored scheduling, and the rewarding experience of caring for medically-fragile patients in the community.

Qualifications

• Current RN license in the state of practice.
• Current health certificate as required by the facility.
• Current PPD or Chest X-Ray.
• Current BLS card.
• One year prior RN experience preferred.

Responsabilités

• Utilizes the nursing process to assess, plan, implement and evaluate patient care.
• Assess signs and symptoms indicating physiologic and psychosocial changes.
• Collects, analyzes, and interprets data from health care members and documents diagnoses.
• Document the plan of care with nursing diagnoses, outcomes and interventions.
• Perform interventions according to priorities, plan of care and hospital policies.
• Revise the plan of care based on evaluation and changes in the medical plan.
• Use clinical judgment in evaluation to meet patient care needs of assigned unit.
• Other RN duties as assigned.

We are seeking a dedicated Registered Nurse to join our Home Health team. to assume responsibility and accountability for the application of the nursing process and the delivery of patient care. The Registered Nurse (RN) demonstrates the ability to make clinical judgments in an effective and efficient manner under the direction of the Director of Clinical Services.

Responsibilities

• Utilizes the nursing process to assess, plan, implement and evaluate patient care.
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.
• Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.
• Document the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions.
• Performs interventions according to identified priorities, plan of care, and the hospital policies and patient care outcome standard.
• Revises the plan of care according to evaluation, changes in medical plan of care, and effective/ineffective nursing interventions.
• Uses clinical judgment in evaluation activities to meet patient care needs of an assigned unit/floor including establishing priorities.
• Other Registered Nurse (RN) duties as assigned.

Requirements

• Current Registered Nurse (RN) License for the state in which the nurse practices.
• Current Health Certificate (per facility Registered Nurse (RN) contract or state regulation).
• Current PPD or Chest X-Ray.
• Current BLS card.
• One year prior Registered Nurse (RN) experience preferred.
