Job summary

Mayo Clinic in Jacksonville, Florida, is seeking a Registered Nurse (RN) to coordinate nursing care, educate patients and families, and support transitions of care. Qualifications

• Graduate of a baccalaureate nursing program.
• One year of RN experience in an applicable care setting or one year of LPN experience at Mayo Clinic is preferred.
• Excellent communication skills (verbal and written).
• Experience working in a team environment.

Responsibilities

• Coordinate nursing care for assigned patients; provide direct patient care and education.
• Support transitions of care and communication across the care continuum.
