The Registered Nurse (RN) Intake Hospice serves as a compassionate clinical resource for patients, families, providers, and referral sources seeking hospice services. This role is responsible for reviewing referrals, conducting clinical assessments, coordinating admissions, and ensuring patients receive timely access to hospice care. The RN Intake Hospice utilizes clinical expertise, critical thinking, and interdisciplinary collaboration to evaluate patient needs, facilitate care transitions, and support patients and families during a significant stage of their healthcare journey.

Current license to practice as a Registered Nurse in state of practice. Valid compact licenses must be transferred to the state of practice within 60 days of the start of employment.
Basic Life Support (BLS) Certification for healthcare providers.
Bachelor's degree in Nursing (BSN). Education must be obtained from an accredited institution. Degree will be verified.
RNs with less than 12 months of experience must obtain their Bachelor of Science in Nursing (BSN) within four (4) years of their hire date.
One year of home health or hospice experience strongly preferred.
Experience with hospice admissions, intake, care coordination, case management, or transitions of care preferred.
Familiarity with hospice eligibility criteria and end-of-life care services preferred.
