Job summary

TwelveStone Health Partners in the Richmond area is seeking a compassionate RN to join our Home Infusion team, delivering 1:1 nursing care in patients' homes with occasional Infusion Center support.

This role focuses on managing IV therapies, patient education, and independent clinical decision-making while maintaining family-centered care and PHI confidentiality. Travel to patient homes is required.

Qualifications

• Bachelor’s or Associate degree in Nursing with active RN license.
• Infusion experience required; minimum 1 year.
• Current CPR certification (BLS minimum).
• Excellent communication and clinical assessment skills.
• Valid driver's license and ability to travel to patient homes.
• Proficiency with EMR and MS Office helpful.

Responsibilities

• Administer IV infusion therapies and manage lines.
• Assess patients before, during, and after infusions.
• Educate patients and caregivers on meds, line care, infection prevention.
• Perform line care, sterile dressing changes, and lab draws.

We are seeking a compassionate and clinically skilled Registered Nurse to join our Home Infusion team in the Richmond, Virginia area. This role provides 1:1 skilled nursing care to patients receiving IV medications, biologics, and specialty therapies in the comfort of their homes, with occasional support in our Infusion Center. What You’ll Do

• Administer IV infusion therapies and manage peripheral and central lines, including PICCs and ports.
• Assess patients before, during, and after infusions and recognize potential adverse reactions or complications.
• Provide patient and caregiver education on medication administration, line care, infection prevention, and symptom monitoring.
• Perform line care, troubleshooting, sterile dressing changes, and lab draws as needed.
• Respond to emergencies within scope and follow established protocols, including anaphylaxis procedures.
• Complete timely, accurate documentation in the EMR.
• Review patient assessments and laboratory results and communicate changes or concerns to physicians, pharmacists, and leadership.
• Coordinate care with prescribers, home infusion pharmacists, patients, caregivers, and other members of the healthcare team.
• Support individualized care planning while respecting each patient's dignity, autonomy, culture, and rights.
• Schedule and coordinate home visits and manage daily travel to patient homes.
• Maintain required equipment, supplies, safety checks, and regulatory documentation.
• Participate in quality initiatives and support referral and provider relationships as needed.

Requirements

Minimum Qualifications:

Education: Bachelor’s or Associate degree in Nursing; current, active and unrestricted RN license in the state of practice and current CPR certification (BLS minimum) required.

Experience: Minimum of one year experience providing nursing care required. Infusion experience required. Telephone, MS Office and computer/keyboard skills required. Previous EMR experience preferred. Must be knowledgeable of operations processes and guidelines for communication and PHI.
