Job summary

Transitions Care seeks a PRN Hospice Visit Registered Nurse to provide compassionate in-home care for patients with serious or terminal illness. You will conduct skilled nursing visits, perform comprehensive assessments, manage symptoms, and coordinate care with the interdisciplinary hospice team.

You will educate and support patients and families on disease progression, comfort-focused care, and end-of-life planning while documenting accurately and complying with hospice, regulatory, and safety

Qualifications

• Licensed RN with hospice/home health experience.
• Ability to educate patients and families on disease progression and end-of-life planning.

Responsibilities

• Conduct skilled nursing visits in patient homes.
• Perform comprehensive assessments of patient condition.
• Manage symptoms and administer medications as prescribed.
