Job summary

Constellation Health Services is seeking a compassionate Hospice Registered Nurse to provide in-home, assisted living, and skilled nursing facility care in the Kennebunk, Maine area. The role is PRN for approximately 16-20 hours per week, with weekend and holiday coverage as needed.

Qualifications

• Graduate of an accredited RN program and valid RN license in ME.
• 1-2 years of RN experience; hospice experience strongly desired.
• Solid knowledge of home health care and patient documentation.

Responsibilities

• Provides skilled nursing care including medications and treatments in the home or facility setting.
• Performs comprehensive assessment and coordinates plan of care with disciplines and families.
• Observes, evaluates and reports changes to nursing supervisor and physician.
• Participates in Interdisciplinary Team Meetings and updates on patient status.
• Maintains documentation per agency standards; uses electronic system for records.
• Supports Home Health Aide Supervisor for hospice aide evaluations.
• Weekend and holidays coverage as needed.
