Job summary

Amedisys Home Health seeks a Registered Nurse to cover the Crossville area (Cumberland County, Fentress County) on a PRN as-needed basis. You will assess patients, develop and update care plans, collaborate with the care team, educate families, and participate in on-call coverage to support patients at home.

This role emphasizes flexible scheduling, distancing care needs, and coordination with the clinical team to ensure high-quality home health care for patients in their familiar surroundings.

Qualifications

• Current RN license for state(s) you are assigned to work.
• CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Performs initial nursing assessments for new admissions.
• Coordinates and revises the plan of care, maintaining continuity of patient care with the care team.
• Regularly communicates patient progress to clinical manager, physician, and care team.
• Maintains patient care records with accuracy and timeliness per policies.
• Records changes in pain/symptom management and outcomes.
• Educates families on patient condition and care needs.
• Makes referrals to other hospice disciplines as needed, or documents rationale not to.
• Performs on-call responsibilities and duties as assigned.
• Other duties as assigned.

• Completes, coordinates, and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff.
• Regularly communicates patient progress to the clinical manager, physician, and care team.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• One year of nursing experience preferred. If less than one year of nursing experience, you can be considered for the RN new graduate program.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
