Job summary

VitalCaring Group seeks a Registered Nurse (RN) - Hospice to lead compassionate, patient- and family-centered end-of-life care in home and community settings. You will develop and manage plans of care, coordinate with the interdisciplinary team, and ensure comfort-focused treatment aligned with patient goals.

Qualifications

  • Active RN license in state or compact
  • Minimum 1 year of clinical nursing experience
  • Proficient in EMR systems
  • Ability to manage a caseload independently
  • Excellent communication in sensitive end-of-life care

Responsibilities

  • Lead Patient-Centered Hospice Care: conduct comprehensive patient and family assessments; develop and manage individualized, comfort-focused care plans; deliver skilled nursing care with pain and symptom management.
  • Serve as the Care Coordinator: act as case manager, coordinating care across the interdisciplinary team; collaborate with physicians, social workers, chaplains, and aides; monitor patient condition and adjust care plans.
  • Support Patients & Families: provide education, emotional support, and guidance; help families navigate complex decisions with empathy and clarity; build meaningful, trusting relationships.
  • Drive Quality & Compliance: maintain accurate, timely documentation; participate in IDT meetings and care planning; support quality and compliance standards.

Role Overview

As a Hospice RN at VitalCaring, you are more than a nurse - you are the clinical leader, advocate, and steady guide for your patients and their families.

You’ll lead the plan of care, manage symptoms, and ensure every patient receives personalized, comfort-focused care aligned with their goals and wishes. Through each visit, you bring not only clinical expertise, but also reassurance, clarity, and compassion.

What You’ll Do
Lead Patient-Centered Hospice Care

What You Bring
  • Active RN license (or compact eligibility)
  • Graduate of an accredited nursing program
  • Minimum 1 year of clinical nursing experience
  • Strong clinical judgment and compassionate care approach
  • Ability to manage a caseload independently
  • Excellent communication skills in sensitive situations
  • Basic EMR proficiency
  • Valid driver’s license and reliable transportation
