Choices Healthcare/Hospice of Central PA seeks an RN to provide skilled nursing care in a home hospice setting. The role requires strong collaboration with the Interdisciplinary Team to meet patient and family needs, with orientation and ongoing training provided.

This full-time day position involves working with patients and caregivers, ensuring high-quality care and compliance with hospice standards. Benefits include comprehensive insurance and retirement plans.

Qualifications

• Must hold a current RN license in good standing.
• Minimum of 1 year RN experience; hospice/home health preferred.
• Ability to collaborate with the Interdisciplinary Team to address patient and caregiver needs.

Responsibilities

• Provide skilled nursing services to address urgent and ongoing needs for patients and caregivers.
• Collaborate with the Interdisciplinary Team to coordinate care.
• Complete orientation; maintain required documentation and patient records.

Home Hospice, FT, Days (8:30am - 5:00pm)

Every 6th weekend (subject to change), 10am - 6pm

2 holidays/year

Works in collaboration with the Interdisciplinary Team to provide skilled nursing services to address urgent and ongoing needs for patients and caregivers. This position requires a year of RN experience. Must be able to complete 5-6 weeks of a FT, day shift orientation.

• Must work a minimum of 64 hours per two-week pay period
• Must work a minimum of 40 hours per two week pay period
