Job summary

CenterWell Home Health in Iowa City / North Liberty is seeking a Home Health Registered Nurse to deliver one-on-one skilled care in patients' homes, guided by the Clinical Manager. You will develop care plans with physicians and monitor recovery to help patients regain independence.

This full-time role requires current RN licensure, CPR, and reliable transportation; driving within the South Region. Competitive pay includes per-visit and base pay with benefits starting Day One.

Qualifications

Responsibilities

• Provide skilled nursing care in patients' homes, including meds, wound care, and assessments.
• Develop and implement individualized care plans with physicians and clinicians.
• Monitor conditions and report changes to physicians and Clinical Manager.
• Educate patients and families on disease management and treatments.
• Maintain accurate care records and coordinate with healthcare teams.
• Report progress to physician and Clinical Manager.
• Oversee Home Health Aides and ensure proper documentation.

As a CenterWell Skilled Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one‑on‑one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you'll manage care plans that support recovery and help patients get back to the life they love.

For this full‑time position providing skilled home care, patients are located in our South Region including the Iowa City and North Liberty areas. Our full‑time benefits start on Day One of employment!

As a Home Health Registered Nurse, You Will

• Provide skilled nursing care to patients, working onsite in their homes, including administering medications, performing wound care, and conducting patient assessments.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
• Monitor patients' conditions and report changes.
• Educate patients and their families on disease management, medication, and treatment options.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Report patient care and condition progress to the patient's physician and Clinical Manager.
• Oversee the work of Home Health Aides and monitor their appropriate completion of documentation.

Required Qualifications

• Med‑surg, ICU, ER, or acute experience.
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver's license, auto insurance, and reliable transportation.
• Live within coverage area of North Liberty / Iowa City patient census.

Preferred Qualifications

• Home Health experience.

Pay Range

• $49.00 - $69.00 pay per visit/unit.
• $77,200 - $106,200 per year base pay.

Additional Information

• TB Statement: This role is considered patient‑facing and is part of Humana's Tuberculosis (TB) screening program. If selected, you will be required to be screened for TB.
• Driving Statement: This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage or whichever is higher.
