As a Registered Nurse at Daymark Health, you will be a key member of an integrated Care Team, which includes a Nurse Practitioner, Health Partner, Enrollment Specialist, and Social Worker. You will deliver hands‑on, compassionate care directly in patients' homes and through virtual platforms. You will provide triage, case management, and care navigation for oncology patients, many of whom have additional comorbidities.
In this role, you will provide direct patient care in the home and through virtual platforms. Additionally, you will conduct health assessments, educate patients, and deliver high-quality care to meet each patient's unique needs. In this role you will:
Experience: A Bachelor of Science Degree in Nursing is required, along with unrestricted registered nurse (RN) compact licensure in MARKET. Additionally, you have at least 3 years of nursing experience, with some prior experience in home care strongly preferred. Experience in oncology, palliative, or hospice care is a plus.
Qualifications: Oncology-related certifications such as Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM) are a plus.
