Job summary

Medical Services of America seeks a Registered Nurse (RN) to join our Home Health team in Jacksonville, FL. This full-time weekend role delivers skilled, patient-centered care in patients' homes under the supervision of the Director of Professional Services.

You'll perform initial assessments, develop care plans, monitor progress, provide family education, and coordinate with physicians. Florida RN license and at least 1 year of home health experience are required.

Qualifications

• Completion of an accredited nursing program.
• Active Registered Nurse license in Florida.
• At least 1 year of clinical nursing experience (home health preferred).

Responsibilities

• Provide one-on-one skilled nursing care to home health patients.
• Perform comprehensive initial assessments and develop individualized care plans with physicians.
• Monitor, evaluate, and document patient progress and adjust care plans.
• Deliver education and support to patients and families for home-based care.
• Ensure care aligns with agency policies, guidelines, and orders.
• Participate in rotating on-call responsibilities to ensure continuity of care.

Hourly Range: $66,560.00 – $80,000.00

We’re currently seeking a dedicated and experienced Full-Time Weekends Registered Nurse (RN) to join our growing team and provide skilled, patient-centered care to individuals in the Jacksonville (Duval, Baker, Clay, Nassau, St. Johns, Flagler, and Volusia), FL areas.

As a Home Health RN, you’ll be a trusted guide in a patient’s recovery journey—offering support, clinical expertise, and compassionate care in the comfort of their home. Working under the direction of the Director of Professional Services, you'll help patients regain independence and improve their quality of life while avoiding unnecessary hospital visits.

What You’ll Do:

What You Bring:

• Reliable transportation and valid driver’s license.
