Job summary

Home Health Care, Inc. is seeking an RN Visit Nurse to provide skilled home health visits across the St. Paul area. The role supports independent field work with strong clinical backing and a collaborative care team.

We offer a $4,000 sign-on bonus and a competitive annual salary. This full-time position includes on-call and weekend rotation and uses Homecare Homebase for documentation.

Qualifications

• Active Minnesota RN license.
• Experience in home health, acute care, rehabilitation, long-term care, or skilled nursing.
• Strong clinical assessment and decision-making skills.
• Excellent communication and patient education abilities.
• Self-motivated with strong organizational skills.
• Reliable transportation for local travel throughout the St. Paul service area.

Responsibilities

• Provide skilled RN home visits throughout the St. Paul territory.
• Complete comprehensive patient assessments.
• Perform wound care, medication management, injections, and other skilled nursing procedures.
• Develop and update individualized care plans.
• Educate patients and caregivers on medications and disease management.
• Communicate with physicians and interdisciplinary care teams.
• Complete timely documentation using Homecare Homebase.
• Monitor patient progress and coordinate ongoing care.
