Vertus HealthCare is seeking experienced Registered Nurses (RNs) to provide high-quality skilled nursing care to patients in their homes throughout Palm Beach County. The role focuses on visiting patients and coordinating with the clinical team to ensure optimal outcomes.

Responsibilities include completing OASIS-related visits, discharge planning, and accurate documentation. Florida RN license and home health experience are preferred, with flexible per diem opportunities available.

Qualifikationen

• Active Florida RN license and eligible to practice in home health.
• Home health experience preferred.
• OASIS experience preferred and accurate charting skills.
• Strong clinical assessment and documentation abilities.
• Reliable transportation to patient homes within Palm Beach County.
