Job summary

CoxHealth is seeking a Home Health RN Case Manager to deliver one-on-one in-home nursing services. You will assess patients in their homes, guide care plans, and coordinate resources to support the patient’s ability to remain at home.

The role emphasizes strong communication, leadership, and time-management skills, with licensure as an RN in Missouri and a background in home health care preferred.

Qualifications

• Graduate of an accredited nursing program or NLN approved program.
• Bachelor’s Degree in Nursing preferred.
• MO RN license active in the state of Missouri.

Responsibilities

• Provide in home evaluation, direction and application of skilled Nursing Services to complement the patient’s Plan of Care.
• Case management ensures education and resources help patients/caregivers manage conditions at home.

Job Description

The Home Health RN Case Manager provides one on one care and skilled services to patients in their own home, contributing to the individual’s overall quality of life. Provides in home evaluation, direction and application of skilled Nursing Services to complement the patient’s overall Plan of Care. Case Management ensures the overall patient condition is cared for by providing appropriate education and resources to the patient/caregivers, allowing them to manager their conditions and remain at home.
