Girling Health Care of New York is seeking an experienced Registered Nurse for home health in Manhattan. This full-time role focuses on planning, coordinating, and directing home care services within patients’ homes and communities.

The RN delivers nursing care, educates patients and families, and collaborates with physicians to ensure comprehensive care. A NYS license is required; vehicle access is necessary; BI-lingual preferred but not mandatory.

Qualifications

• Graduate of an accredited school of nursing; ADN or BSN
• NY State license and registration to practice as a Registered Professional Nurse in New York State
• Homecare experience preferred
• Must have a vehicle to travel to patients
• Bi-lingual preferred (not required)

Responsabilités

• Complete initial patient and family assessment to determine home care needs.
• Provide comprehensive physical assessments and histories.
• Regularly re-evaluate patient nursing needs and adjust plans.
• Initiate and revise the care plan based on patient status.
• Administer medications and treatments as prescribed by physician.
• Counsel patient and family on nursing needs and care.
• Provide health care instructions per assessment and plan of care.
• Identify discharge planning needs and integrate before discharge.
• Act as Case Manager as assigned by Clinical Supervisor.
• Prepare clinical notes and notify primary physician periodically.

Girling Health Care of NY is looking for experienced and reliable Registered Nurses to conduct home visits in NYC. The registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

• Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
• Counsels the patient and family in meeting nursing and related needs.
• Provides health care instructions to the patient as appropriate per assessment and plan of care.
• Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
• Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
• Prepares clinical notes and updates the primary physician when necessary and at least every sixty days.

Nurse Requirements:

• Graduate of an accredited school of nursing; ADN or BSN
• Homecare experience is strongly preferred
• Must have current NYS license and registration to practice as a Registered Professional Nurse in New York State
• Must have a vehicle to travel to patients
• Bi-Lingual preferred (but, not a requirement)

FT Salary Range: $100K - $115K

Fee For Service (Per Diem): $85 per visit
