Job summary

Elara Caring is seeking a Registered Nurse Home Health in Michigan to provide patient-centered care in patients’ homes. This full-time role includes a $5,000 sign-on bonus and uses new ambient listening tech to save time on documentation, allowing focus on patient needs.

Responsibilities include assessments, planning care, HIPAA privacy, and collaborating with the care team to ensure quality, safe, ongoing patient care. Valid RN license and 1+ year experience required.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including transferring patients and moving equipment.

Responsibilities

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient and staff privacy in accordance with HIPAA.
• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.
• Educating patients and families on their care and safety.
• Taking appropriate action when patient conditions change.
• Helping ensure quality, safe, and continuous patient care.
