Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) Admissions or Case Manager for full-time or part-time roles serving adult clients in their homes across Kent County, Delaware, including Smyrna, Clayton and Dover.

As a home health RN, you join a multidisciplinary team to deliver rehabilitative care, medications, wound care, and coordinated services while maintaining clear documentation in our EMR system.

Qualifications

• RN license must be active in Delaware.
• 1 year of clinical experience (medical/surgical) preferred.
• Home health experience preferred but not required.
• Graduate from an accredited nursing program with transcript/diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills and communication abilities.
• Solid computer skills; experience with EMR preferred.
• Ability to travel to cases as assigned.

Responsibilities

• Follow designated care plans for patient needs.
• Make home visits to clients in designated geographic territories.
• Administer medications, perform wound care, treatments, and procedures.
• Monitor client conditions and report changes to supervisor.
• Follow up on doctors’ orders and document actions.
• Perform client assessments as necessary and coordinate case management.
• Accurately document observations, interventions, and evaluations.

As a home care Registered Nurse , you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

Registered Nurse (RN) Responsibilities:

• Follow a designated care plan in accordance with patient’s needs
• Makehome visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients’ conditions; reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors’ orders
• Perform client assessments as necessary
• Case management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a Registered Nurse (RN):

• A current license as a Registered Nurse in DE.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred, but not required.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.
